Physical Therapist

Location: Tuba City, Arizona
Employment Type: Contract
Contract Length: 13 weeks

Job Description

Provide quality physical therapy services focused on treatment or prevention of physical disability or disease. Responsibilities include:

• Management and implementation of the Physical Therapy program
• Evaluation and treatment of inpatients and outpatients, ages birth through geriatric
• Specialty services such as wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation/treatment, and Durable Medical Equipment Prosthetic Orthotic Supply Program
• Direction and development of the physical therapy section including quality improvement, risk management, safety, policies and procedures
• Participation in staff education, reporting, meetings, in-services, and Joint Commission preparation
• Compliance with Medical Staff Credentialing Core Privileges requirements
• Weekend rotation required

Required Qualifications

• Bachelor’s degree from a CAPTE-accredited school of Physical Therapy
• Valid, current, full, and unrestricted Physical Therapist license in any U.S. state, District of Columbia, or Puerto Rico during employment
• Current Basic Life Support (BLS) certification by the American Heart Association
• Must meet credentialing requirements to serve on the Medical Staff (including fingerprinting clearance prior to start)

Additional Information

• Orientation occurs every 2 weeks
• COVID & Flu vaccination exemptions may be accepted after submission of a declination form
• Housing application must be submitted 2 weeks prior to start
