Physical Therapist - Outpatient

Location: International Falls, Minnesota
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Summary

Plans, organizes, and implements specific treatment programs for individual patients according to the principles and practices of Physical Therapy. Collaborates with an interdisciplinary treatment team.

Schedule

• Monday to Friday
• 8:00 AM to 5:00 PM
• No weekends

Patient Load

• 10-13 patients per day
• 40 guaranteed hours per week

Specialty Focus

• Acute Care: 80-90% general outpatient PT
• 10-20% inpatient/swing bed

Required Qualifications

• Valid Physical Therapist license in Minnesota
• Minimum 1 year of Physical Therapist experience, including experience within the last year
• Bachelor’s degree in Physical Therapy
• Basic Life Support (BLS) certification
• Experience in both inpatient and outpatient settings

Required Certifications & Documents

• Basic Life Support (BLS)
• Physical exam within 1 year of start
• Negative TB test or chest X-ray within 1 year
• Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella
• 9-panel drug screen within 1 year
• Flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Background checks including SS trace, county searches, nationwide sex offender check (within 1 year)
• Emergency contact form
• Professional references (2 for travel/allied/per diem acute care; 1 for per diem non-acute care)
• Competency exams as applicable
