Job Opportunity: Home Health Registered Nurse (RN) - Case Manager
Location: Saint Louis, Missouri
Employment Type: Travel / Contract
Contract Length: 13 weeks
Job Details
- Schedule: Monday - Friday, 8:00 AM to 4:30 PM
- Patient Load: 4-6 patients per day
- Visits: Approximately 25-30 visits per week to total 40 hours (point system)
- Types of Cases: Home Health, Wound Care, Geriatric Home Health Patients
- On-Call: Discretionary (not required)
- Weekend Requirement: No weekends required
- Mileage Reimbursement: IRS standard (from first patient to last patient, not to/from home)
- Locals Accepted: Yes, within a 50-mile radius
- Dress Code: Blue scrubs, closed-toe shoes
- Charting: Must be completed at time of visit using Matrix system; no charting allowed to be taken home
- Parking: Free parking available
- COVID & Flu Policy: Flu vaccination required or mask must be worn while patient-facing
- Compliance: All required documents must be submitted before starting; compliance processing may take 2-3 weeks
- Return to Office (RTO): All RTO documents must be submitted at time of application; no additions accepted after offer
Required Qualifications
- Valid RN license in Missouri
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 2 years of Home Health nursing experience, including experience within the last year
- At least 1 year of clinical management experience in a Home Health setting
- Current BLS certification
