Job Opportunity: Home Health Registered Nurse (RN) - Case Manager

Location: Saint Louis, Missouri
Employment Type: Travel / Contract
Contract Length: 13 weeks

Job Details

• Schedule: Monday - Friday, 8:00 AM to 4:30 PM
• Patient Load: 4-6 patients per day
• Visits: Approximately 25-30 visits per week to total 40 hours (point system)
• Types of Cases: Home Health, Wound Care, Geriatric Home Health Patients
• On-Call: Discretionary (not required)
• Weekend Requirement: No weekends required
• Mileage Reimbursement: IRS standard (from first patient to last patient, not to/from home)
• Locals Accepted: Yes, within a 50-mile radius
• Dress Code: Blue scrubs, closed-toe shoes
• Charting: Must be completed at time of visit using Matrix system; no charting allowed to be taken home
• Parking: Free parking available
• COVID & Flu Policy: Flu vaccination required or mask must be worn while patient-facing
• Compliance: All required documents must be submitted before starting; compliance processing may take 2-3 weeks
• Return to Office (RTO): All RTO documents must be submitted at time of application; no additions accepted after offer

Required Qualifications

• Valid RN license in Missouri
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 2 years of Home Health nursing experience, including experience within the last year
• At least 1 year of clinical management experience in a Home Health setting
• Current BLS certification
