Job Opportunity: Physical Therapist - Home Health

Job Description

• Provide physical therapy services in patients' homes (home visits only).
• Average caseload: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day.
• Types of cases: Start of Care and Follow-Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (some locations may have alternate schedules such as 4x10 shifts Friday-Monday).
• Must have reliable transportation and a current driver's license.
• Willingness to travel up to 75 miles from the home clinic, covering multiple counties.
• Mileage reimbursed at IRS standard rate (compensated from patient home to patient home, not commute to/from first/last patient).
• Local candidates accepted.
• On-call requirements vary by location; discuss during interview.
• Weekend requirements vary; discuss during interview.
• Opportunity for hire at the end of the contract if interested.

Required Qualifications

• Valid Physical Therapist license in Georgia.
• Minimum of 1 year of recent experience as a Physical Therapist in an Adult Home Health setting.
• Experience working with adult patients.
• Current BLS certification.
• Reliable transportation and valid driver's license.
• Must carry agency-required liability insurance.

Productivity Expectations

• Weekly productivity units: 25-30 units.
• Travel approximately 30-40 miles per day.

Additional Requirements & Compliance

• Background checks including 7-year history, sex offender, and state-specific searches as applicable.
• Drug screening (10-panel) required.
• Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella as per standard healthcare requirements.
• COVID-19 vaccination series or declination required.
• Various state-specific and facility-required documents and attestations must be completed prior to start.
• Candidates must review and sign compliance, confidentiality, privacy, and policy documents prior to assignment start.

Travel Experience

• Prior travel experience is not explicitly required, but recent home health experience is mandatory.
