Outpatient Physical Therapist - Travel/Contract

Job Details

• Shift: Monday to Friday, either 5 days x 8 hours or 4 days x 10 hours
• Daily Patient Load: Approximately 11 patients
• Guaranteed Hours: 40 hours per week
• Weekend Requirement: As needed
• On Call: No call required
• Floating: May be asked to float to surrounding outpatient clinics
• Blocked Scheduling: No
• Locals Accepted: No
• RTO Restrictions: Unit Manager’s approval required
• Facility Type: Hospital-based ambulatory clinic supporting Orthopedics, Geriatrics, Cardiopulmonary, Neurological, Lymphedema, and Pelvic Health

Required Qualifications

• Valid Physical Therapist license in Arizona
• Minimum 1 year of experience as a Physical Therapist, including experience within the last year
• Minimum 1 year of experience in Outpatient Physical Therapy
• BLS (Basic Life Support) certification issued by the American Heart Association

Additional Requirements

• Must complete a 7-year background check within 30 days of start
• Required to complete various competency exams and health screenings including:

  • Medication/Pharmacology competency exam
  • Color blindness testing (if applicable)
  • Physical exam within 1 year of start confirming ability to perform essential job functions
  • Fit mask test (N95) within 1 year and updated annually (not required for outpatient PTs but may be requested)
  • Immunizations and titers: Tdap, Measles, Mumps, Rubella, Hepatitis B, Varicella, Flu vaccine or declination, COVID vaccine or declination
  • Negative TB test or chest x-ray within 1 year and updated annually
  • Drug screening (10 panel) within 30 days of start and updated annually
• Must complete facility-specific job description form prior to start
• Must adhere to professional attire and appearance policies
• Must sign confidentiality and substance abuse screening policies prior to start
• Two professional references required (one from most recent employer within 1 year)
• Employment application required with 3 years of work history and explanation of gaps over 30 days

Work Environment

• Short-term acute care hospital-based outpatient clinic
• Teaching facility with Level IV trauma designation
• Total staffed beds: 87
• Business casual dress code
• Electronic charting system used
• Free parking available
• Adherence to quality and patient safety standards including medication administration protocols

Notes

• No prior travel experience required
• May be asked to float to other outpatient clinics within the area
• No call or weekend guaranteed shifts, but weekend work may be needed occasionally
