Job Description

As a Primary RN Case Manager in hospice care, you will provide compassionate, patient-centered care that supports patients and their families during challenging times. This role involves:

• Delivering expert symptom management and holistic care across a variety of settings including private homes, nursing homes, group homes, and assisted living facilities.
• Acting as a generalist in all diseases with a specialty in symptom management.
• Leading an interdisciplinary team to coordinate care for an assigned caseload of patients.
• Teaching and supporting patients and caregivers to identify personal goals of care and build confidence in caregiving.
• Working primarily Monday through Friday, 8am-5pm, with occasional weekend backup and on-call duties.

Required Qualifications

• Valid Registered Nurse (RN) license in Colorado
• Minimum of 2 years of hospice nursing experience, including experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

• Prior travel experience is not required for this position.
• The role emphasizes compassionate care, teaching, and leadership within an interdisciplinary team.
• Schedule is primarily weekday with some flexibility for weekend coverage.
