Job Description

Provide and coordinate home nursing care according to physician orders and agency policies.
Directly supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
Manage an average caseload of 5-8 patients per day, including start of care, wound care, and follow-up visits.
Work primarily Monday through Friday, 8:00 AM to 5:00 PM, with on-call responsibilities during the week.
Travel up to 75 miles from the home clinic; weekly travel estimated between 150-500 miles.
Use HomeCare HomeBase (HCHB) for charting; proficiency in OASIS required.
Dress code: Scrubs.

Required Qualifications

Valid Registered Nurse (RN) license in Missouri.
Minimum of 1 year of hospice nursing experience within the last year.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Proficiency with HomeCare HomeBase (HCHB) and OASIS documentation systems.
