Job Description

• Must complete documentation directly with the therapy provider before starting the assignment.
• Candidates can start with a compact license but must obtain a permanent license immediately.
• Candidates with a permanent license must have a permanent license to start; compact license not accepted for permanent positions.
• Candidates who have worked permanently or PRN within the last year at this provider are not eligible to return until separated for one year.
• Previous travelers or permanent/PRN workers from more than one year ago require eligibility verification.
• Equipment will be provided during the interview.
• Parking will be provided on the first day.
• Dress code details will be provided on the first day.
• 32 hours guaranteed work week during holiday weeks.
• Overtime paid at 1.5x.
• 30-day notice required.

Required Qualifications

• Valid Physical Therapist license in Arizona
