Unit & Shift Details
- Unit: Intermediate Care Unit (Telemetry)
- Shift: Day shift, 3 shifts per week, 12 hours each (7:00 AM - 7:30 PM)
- Hours per week: 36 hours (for 3x12 shifts)
- Nurse-to-patient ratio: 1:5-6
- Weekend requirement: Every other weekend
- On-call required
- Floating within scope to meet facility needs
Experience & Requirements
- Valid Registered Nurse (RN) license in Illinois
- Minimum of 1 year of telemetry nursing experience, with telemetry experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- ACLS certification issued by the American Heart Association
- BLS certification
- NIH Stroke Scale (NIHSS) certification
Certifications Required
- ACLS (Advanced Cardiac Life Support) - AHA only; certificates of completion accepted for up to 1 month until official card is issued
- BLS (Basic Life Support) - AHA only; certificates of completion accepted for up to 1 month
- NIH Stroke Certification (annual update required unless 2-year expiration listed)
- Other certifications may be required based on unit specifics (refer to unit descriptions)
Licensure
- Active Registered Nurse license for Illinois required
- Primary source verification of professional license required
- NURSYS verification required for Missouri and Illinois licenses
- Illinois temporary licenses not accepted; permanent Illinois license required for assignment extension
Scheduling & Work Conditions
- Block scheduling not accommodated
- Floating within scope expected
- Guaranteed hours: 36 hours per week for 3x12 shifts

Facility & Documentation Requirements
- Acute care hospital with 158 staffed beds
- Charting system: EPIC
- Parking: Free
Compliance & Background Checks
- Government-issued ID required (e.g., driver's license or state ID)
- Criminal background check within 6 months required (7-year search including statewide, nationwide, and international if applicable)
- EPLS/GSA/SAM and HHS/OIG checks required quarterly
- Fingerprinting required for assignments at or floating to long-term care or rehab units in Illinois
- Education verification of highest nursing degree required (cannot pend at start)
- State-specific exclusion and sanction list checks required and must be renewed quarterly
- Drug testing within 1 year of start, including a comprehensive panel (amphetamines, barbiturates, benzodiazepines, cocaine, methadone, opiates, oxycodone, PCP), must not expire during assignment
- Physical exam within 1 year of start required
- Fit mask testing (N95) required and must be renewed annually
- Immunizations and health screenings required:
- Tdap vaccine or declination (expires every 10 years)
- Negative TB test or chest x-ray per protocol
- Measles, Mumps, Rubella, Varicella immunity or vaccination per policy
- Seasonal flu vaccination or approved exemption required during flu season (Sept-April)
- Various facility-specific documents required prior to start, including:
- HIPAA Acknowledgement
- Latex Allergy Questionnaire
- Confidentiality Form
- Regulatory Compliance Education (annual quiz with 80% passing score)
- TB Questionnaire
- Hand Hygiene and Fingernail Policies
- Dress Code Policy
- Orientation Modules (approx. 16 hours, completed via online platform; required for all RNs and LVNs)
Additional Requirements
- Two professional references required (supervisors preferred within last year)
- Unit-specific competency exams and skills checklists required and must be updated annually
- Must comply with all facility policies and complete required training modules before start
