Unit & Shift Details

  • Unit: Intermediate Care Unit (Telemetry)
  • Shift: Day shift, 3 shifts per week, 12 hours each (7:00 AM - 7:30 PM)
  • Hours per week: 36 hours (for 3x12 shifts)
  • Nurse-to-patient ratio: 1:5-6
  • Weekend requirement: Every other weekend
  • On-call required
  • Floating within scope to meet facility needs

Experience & Requirements

  • Valid Registered Nurse (RN) license in Illinois
  • Minimum of 1 year of telemetry nursing experience, with telemetry experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • ACLS certification issued by the American Heart Association
  • BLS certification
  • NIH Stroke Scale (NIHSS) certification

Certifications Required

  • ACLS (Advanced Cardiac Life Support) - AHA only; certificates of completion accepted for up to 1 month until official card is issued
  • BLS (Basic Life Support) - AHA only; certificates of completion accepted for up to 1 month
  • NIH Stroke Certification (annual update required unless 2-year expiration listed)
  • Other certifications may be required based on unit specifics (refer to unit descriptions)

Licensure

  • Active Registered Nurse license for Illinois required
  • Primary source verification of professional license required
  • NURSYS verification required for Missouri and Illinois licenses
  • Illinois temporary licenses not accepted; permanent Illinois license required for assignment extension

Scheduling & Work Conditions

  • Block scheduling not accommodated
  • Floating within scope expected
  • Guaranteed hours: 36 hours per week for 3x12 shifts
  • Scrub color: Navy

Facility & Documentation Requirements

  • Acute care hospital with 158 staffed beds
  • Charting system: EPIC
  • Parking: Free

Compliance & Background Checks

  • Government-issued ID required (e.g., driver's license or state ID)
  • Criminal background check within 6 months required (7-year search including statewide, nationwide, and international if applicable)
  • EPLS/GSA/SAM and HHS/OIG checks required quarterly
  • Fingerprinting required for assignments at or floating to long-term care or rehab units in Illinois
  • Education verification of highest nursing degree required (cannot pend at start)
  • State-specific exclusion and sanction list checks required and must be renewed quarterly
  • Drug testing within 1 year of start, including a comprehensive panel (amphetamines, barbiturates, benzodiazepines, cocaine, methadone, opiates, oxycodone, PCP), must not expire during assignment
  • Physical exam within 1 year of start required
  • Fit mask testing (N95) required and must be renewed annually
  • Immunizations and health screenings required:
    • Tdap vaccine or declination (expires every 10 years)
    • Negative TB test or chest x-ray per protocol
    • Measles, Mumps, Rubella, Varicella immunity or vaccination per policy
    • Seasonal flu vaccination or approved exemption required during flu season (Sept-April)
  • Various facility-specific documents required prior to start, including:
    • HIPAA Acknowledgement
    • Latex Allergy Questionnaire
    • Confidentiality Form
    • Regulatory Compliance Education (annual quiz with 80% passing score)
    • TB Questionnaire
    • Hand Hygiene and Fingernail Policies
    • Dress Code Policy
    • Orientation Modules (approx. 16 hours, completed via online platform; required for all RNs and LVNs)

Additional Requirements

  • Two professional references required (supervisors preferred within last year)
  • Unit-specific competency exams and skills checklists required and must be updated annually
  • Must comply with all facility policies and complete required training modules before start
