Outpatient Physical Therapist - Travel/Contract

Location: Sedona, Arizona
Contract Length: 13 weeks

Job Details

• Shift: Monday to Friday, 8:00 AM – 4:30 PM (5 days x 8 hours)
• Hours per Week: 40 hours guaranteed
• Daily Patient Load: Approximately 11 patients
• Weekend Requirement: As needed
• On Call: No call required
• Floating: May be asked to float to surrounding outpatient clinics
• Scheduling: No blocked scheduling
• Dress Code: Business casual

• Adherence to quality and patient safety standards including medication administration (BCMA) expected

Required Qualifications

• Valid Physical Therapist license in Arizona
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association

Compliance & Documentation Requirements

• Valid driver’s license or state photo ID required (passport alone not accepted)
• Background checks including 7-year history, SSN trace, criminal, and sex offender searches required within 30 days of start
• Employment application covering last 3 years with documented gaps over 30 days required
• Two professional references required (one from most recent employer within 1 year)
• Physical exam within 1 year confirming ability to perform essential job functions
• Drug screening (10-panel) within 30 days of start and updated annually
• Immunizations and titers required or declinations accepted for:

  • Tetanus/Diphtheria/Tdap (preferred within 10 years)
  • Measles, Mumps, Rubella (positive titer or 2 MMR vaccines)
  • Hepatitis B (positive titer, vaccine series, or declination)
  • Varicella (positive titer or 2 vaccines)
  • Influenza vaccine or facility-specific declination form (mask required if declined)
  • COVID vaccine series or declination with facility-specific form
• Negative TB test (PPD, Q Gold, or Tspot) within 1 year, updated annually; chest x-ray and TB questionnaire if previously positive
• Fit mask (N95) required within 1 year and updated annually; onsite fit testing available if needed
• Completion of facility-specific job description and policies including confidentiality, substance abuse screening, professional attire, and security questions prior to start

Additional Notes

• Locals are not accepted for this travel assignment
• Facility may call off up to 6 days during the 13-week assignment with unit manager approval required for return to office (RTO) restrictions
• Candidate must be willing to comply with all facility-specific competency exams and testing requirements
• No radiation badge required for this role
