Physical Therapist Job Opportunity

Contract Length: 13 weeks
Shift: 8:30 AM – 5:00 PM
Hours per Week: 40

Required Qualifications

• Valid Physical Therapist license in Pennsylvania
• Allied License/Certification
• Basic Life Support (BLS) certification

Required Screenings and Verifications

• Background check
• Drug screening
• Educational and employment verification
• License verification
• Physical exam
• Immunizations and health screenings including:

  • Hepatitis B
  • MMR (Measles, Mumps, Rubella)
  • TdaP vaccine
  • Varicella
  • TB screening and annual TB questionnaire
  • Latex health questionnaire
  • Color vision screening
• Competency exam and skills checklist
• Core mandatory annual training
• Motor vehicle report and valid driver’s license with car insurance
• Various state and federal exclusion and sanction screenings (e.g., FACIS 3M, NJ Debarment, OFAC Sanction, SAM Sanction, PA Medicheck, PATCH, NJ Sex Offender Exclusion)
