Description

Pharmacist Inpatient (Travel/Contract) - Chicago, IL

Position Overview

  • Role: Licensed Pharmacist providing inpatient pediatric pharmacy services.
  • Setting: Pediatric hospital environment.
  • Shift: Night shift, 9:00 PM to 7:30 AM (4 shifts per week, 10 hours each).
  • Schedule: Seven days on, seven days off.
  • Contract Length: 12 weeks.

Key Responsibilities

  • Ensure accuracy and appropriateness of all medication orders by evaluating drug selection, dose, frequency, safety, dosage form, allergies, drug interactions, and disease state interactions before dispensing.
  • Evaluate patient-specific information such as age, weight, medical characteristics, and lab results to ensure proper medication use.
  • Provide age-appropriate pharmaceutical care based on principles of growth and development.
  • Supervise pharmacy technicians in tasks including:
    • Preparation of first and missing doses.
    • Triaging workflow and order entry.
    • Delivery of STAT medications.
    • Stock replacement and short-list preparation.
    • IV therapy coordination.
    • Nursing unit inspections (checking expired meds, refrigerator checks, hood cleaning).
    • Medication returns and Omnicell restocking.
  • Communicate overnight shift issues and staff call-outs to management.

Required Qualifications

  • Valid Pharmacist license in Illinois.
  • Minimum of 2 years of pharmacist experience.
  • Pharmacist experience within the last year.
  • Experience working with pediatric patients.

Certifications & Competencies

  • Completion of an ASHP-accredited residency is highly preferred.
  • Medication Test/Pharmacology Competency Exam as applicable.
  • Color Blind Testing (Ishihara 14 plate) required.
  • Unit-specific competency exams required and must be updated every 3 years.
  • Annual Core training (Parts I, II, III) and OSHA mandatory training.
  • Basic Life Support (BLS) - AHA only.
  • Pediatric Advanced Life Support (PALS) - AHA only.
  • Neonatal Resuscitation Program (NRP) required for relevant pediatric areas.
  • Advanced Cardiac Life Support (ACLS) - AHA only.

Health & Background Requirements

  • Criminal background check within 6 months prior to start (7-year search including alias, county, and national sex offender checks).
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly.
  • Physical exam within 1 year of start confirming fitness for duty.
  • N95 mask fit testing within 1 year of start.
  • Immunizations and titers required or documentation for:
    • Tuberculosis screening (PPD or Quantiferon Gold).
    • Tetanus/Diphtheria/Tdap within 10 years.
    • Measles, Mumps, Rubella, Varicella (positive titer, vaccination, or physician documentation).
    • Hepatitis B vaccination series or declination.
  • Negative 9-panel drug screen within 1 year of start.
  • Seasonal flu vaccination or approved medical/religious exemption required during flu season.

Additional Requirements

  • Two professional references required; at least one must be from a direct supervisor.
  • Employment application and unit-specific skills checklist must be submitted.
  • Facility-specific documents including badge photo and compliance checklists must be completed prior to orientation.
  • Completion of facility online training modules prior to orientation.
