Description

Registered Nurse - Hospice Case Manager

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks
Work Schedule: Monday - Friday, 36 hours per week

Job Description

Provide hospice care in a home setting as part of hospice case management.
Experience with hospice case management is required.
Prior experience with rural travel is preferred.
Experience using McKesson/Netsmart EMR systems is preferred.
Preference for candidates without Return to Office (RTO) requests for this contract.

Required Qualifications

Valid Registered Nurse (RN) license in Vermont.
Minimum of 1 year of case management experience, including experience within the last year.
Basic Life Support (BLS) certification.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Experience in a home health setting.
Hospice care experience.

Additional Information

This position involves travel to rural areas.
The role supports patients receiving hospice care in their homes.
