Case Manager (Registered Nurse) - Oregon, Hillsboro

Position Details

Employment Type: Travel/Contract
Shift: Day shift, 4x10 hour shifts (7:00 AM - 5:30 PM)
Schedule: Variable with weekend rotations; full weekend off every third weekend
Work Environment: Business casual or scrubs
Location: Hillsboro, Oregon

Job Summary

Analyze patient medical records for quality assessment and discharge planning
Provide documentation to insurance carriers for service authorizations
Coordinate discharge plans including community service needs and transition arrangements
Requires creativity, problem-solving, multitasking, prioritization, and ability to work with minimal supervision

Required Qualifications

Education

Bachelor of Science in Nursing

Experience

Minimum 1 year of Case Management experience within the last year
Experience in an inpatient setting with acute hospital case management

Licenses & Certifications

Valid Registered Nurse license in Oregon
Basic Life Support (BLS) certification issued by the American Heart Association
