RN Case Manager - Home Health

Location: White River Junction, Vermont (also serving surrounding VT and NH areas)

Job Description

• Provide comprehensive home health nursing care as an RN Case Manager.
• Screen, evaluate, and re-evaluate patients; develop and periodically review care plans in collaboration with physicians, patients, and caregivers.
• Coordinate patient care through interdisciplinary communication and case conferences.
• Monitor patients’ clinical, functional, and emotional status; communicate significant changes to physicians and team members.
• Document evaluations, treatment goals, and objectives.
• Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.
• Work with broad independence while being part of a cohesive patient-care team.
• Maintain a work/life balance with flexible scheduling.

Required Qualifications

• Valid Registered Nurse (RN) license in Vermont
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 2 years of experience in Home Health nursing, including experience within the last year
• Experience working in a Home Health setting

Additional Information

• Full-time position with day shifts.
• Service area includes multiple towns in Vermont and New Hampshire.
