Description
RN Care Manager (Contract)
Location: York, Maine
Contract Length: 13 weeks
Hours: 40 hours per week
Shift: Variable shifts: either 8:30 AM – 4:30 PM or four 10-hour shifts from 7:30 AM – 5:00 PM
Call: No call required
Schedule: Weekend and holiday rotation required
Job Summary
The RN Care Manager coordinates concurrent utilization management activities hospital-wide and serves as the primary liaison between the patient, healthcare team, organization, and payors during the patient’s hospital stay.
Key Responsibilities
- Assess patient needs and identify resources required for transition to the next care setting.
- Secure necessary resources for patient transitions.
- Collaborate with and lead an interdisciplinary team during daily care rounds, care conferences, and care planning.
- Communicate effectively in written, verbal, and electronic formats.
- Work with community organizations to arrange durable medical equipment, health insurance authorizations, and transportation.
- Participate in managing the length of stay for inpatient hospital populations.
- Apply knowledge of hospital reimbursement methods and quality measures relevant to care management.
Required Qualifications
- Valid RN license in Maine
- Case Management (3 years of experience)
- Case Management experience within the last year
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in a Home Care setting
- Utilization Review (InterQual)
- Discharge Planning
- Care Manager experience
- Utilization Review experience
- Med Surg experience
Additional Information
- Proper completion of timecards is mandatory; improperly filled timecards will be rejected and not processed.
- Weekend and holiday rotation is part of the schedule.
