Home RN Dialysis

Location: Cooperstown, New York
Employment Type: Long-Term Assignment (13 weeks)
Shift: 4 x 10-hour shifts, 6:00 AM – 4:30 PM

Job Overview

• Provide nursing care for home dialysis patients primarily receiving Peritoneal Dialysis (PD) and Home Hemodialysis (HHD).
• Manage outpatient home dialysis clinics and conduct patient assessments and ongoing clinical management.
• Educate and train new home dialysis patients on their modality.
• Troubleshoot and triage patient concerns and treatment-related issues.
• Coordinate care to support patient independence in the home setting.
• Occasionally assist inpatient nursing units with Continuous Ambulatory Peritoneal Dialysis (CAPD) exchanges.
• Perform initial home visits for new patients as needed.

Patient Population

• Approximately 14 home dialysis patients across multiple clinic locations.
• Majority of patients utilize Peritoneal Dialysis (PD).

Travel Requirements

• Travel between clinic locations may be required.
• RNs travel to patient homes only for initial home assessments/visits.
• Ongoing patient care is primarily clinic-based.
• Personal vehicle required.

On-Call Requirements

• Weekday call coverage: 4:30 PM – 6:00 AM.
• Weekend call coverage: 24-hour coverage Saturday and Sunday.
• Call rotation typically every other week.

Orientation

• Shadowing an experienced team member during the first week.
• Expected to independently manage assigned patients after orientation.

Required Qualifications

• Valid Registered Nurse (RN) license in New York.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Minimum of 1 year of outpatient dialysis experience, with experience within the last year required.
• Basic Life Support (BLS) certification.
