Description

Physical Therapist - Contract Position

Location: Rutland, Vermont
Contract Length: 13 weeks
Shift: Monday to Friday, 8:00 AM to 4:00 PM, with 2-3 weekend days
Hours per Week: 37 hours


Job Description

  • Manage and adjust patient visit schedules to maximize productivity and meet patient needs, including rescheduling and communicating changes.
  • Perform comprehensive patient assessments (including OASIS and physical therapy evaluations) covering psychosocial, educational, safety, and risk factors.
  • Develop individualized plans of care aimed at promoting functional independence and safety, with patient-specific goals and logical interventions.
  • Provide skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education, while assessing patient progress on each visit.
  • Coordinate patient care as a Care Manager or team member with physicians, nurses, caregivers, rehab services, and students to ensure optimal care and communication.
  • Supervise and direct clinical activities of Physical Therapist Assistants (PTAs), PT students, and home care aides.
  • Complete and coordinate required documentation and paperwork to reflect care provided, outcomes achieved, and support billing processes in compliance with federal and state regulations and professional guidelines.
  • Participate in in-service training programs for rehabilitation, medical, nursing staff, volunteers, and community groups.
  • Maintain and enhance professional knowledge and promote physical therapy standards.
  • Engage in marketing and promotion of rehabilitation services, including developing specialized service areas and participating in community health events.
  • Perform patient-related health assessments such as PT/INR and blood pressure monitoring.
  • Conduct patient-related procedures including staple/suture removal and wound care.
  • Communicate changes in patient care schedules promptly to schedulers and supervisors.
  • Complete OASIS time points and collaborate with Quality Improvement staff to ensure accuracy of clinical notes and documentation.
  • Follow agency protocols for visit documentation and billing processes.
  • Accurately complete various agency, state, and federal forms (e.g., HHABN, Medicare Notice of Non-Coverage) in a timely manner.
  • Conduct visits efficiently, ensuring supervisory visits are not combined with routine visits when necessary.
  • Assess patient needs for Home Care Aide (HCA) services, develop and coordinate plans of care, and provide ongoing supervision as required.
  • Coordinate safe patient discharges and transfers, ensuring all paperwork and patient information comply with policies and regulations.
  • Report patient incidents (infections, injuries, etc.) promptly to Quality Improvement specialists.
  • Participate in regular peer review audits, providing and accepting constructive feedback professionally.

Required Qualifications

  • Valid Physical Therapist license in Vermont
  • Minimum of 2 years of experience as a Physical Therapist
  • Physical Therapist experience within the last year
  • Basic Life Support (BLS) certification

Additional Information

  • Prior travel experience is required for this position.
  • Coverage includes Rutland and Bennington counties.
