Job Details
- Specialty: Operating Room (OR) Registered Nurse

Required Qualifications
- Valid RN license in Idaho
- Minimum of 1 year of Operating Room (OR) experience within the last year
- BLS certification issued by the American Heart Association
- ACLS certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Health and Safety Requirements
- Fit Test
- 10-panel drug screen completed within 30 days of start (no THC).
- COVID-19 vaccine proof including booster or approved declination/exemption.
- MMR: Proof of two vaccinations or immunity titer.
- Hepatitis B: Must have started vaccination series prior to start.
- Influenza vaccine: Optional.
- 2-Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB.
- Varicella: Proof of vaccination or immunity.
- Physical/Health Statement confirming fitness to work within the last year.
Professional and Compliance Requirements
- Signed Code of Conduct form.
- Signed Confidentiality and System Security Agreement.
- Specialty-specific competency testing completed within the last year.
- Criminal background check (federal, nationwide, county) within last 7 years, dated within 30 days of start.
- Education verification of highest academic degree or diploma.
- EPLS (Excluded Parties List System) check dated within 30 days.
- OIG (Office of Inspector General) exclusion check dated within 30 days.
- Integrity and Audit Services form signed shortly before start.
- Joint Commission CORE exam within the last year.
- NSO (National Sex Offender) check dated within 30 days.
- System Access form completed and uploaded one week before start.
- Skills checklist provided by agency for clinical contractors.
Additional Notes
- All required documents and verifications must be uploaded to the credentialing system prior to onboarding.
- License and certification verifications must include scanned copies showing front and back of cards.
- Specialty competency exam must be current within one year.
- Drug screen must be completed and resulted before start with Medical Review Officer (MRO) signature.
