Description
Physical Therapist - Home Health (Travel/Contract)
Location: Dunedin, Florida
Contract Length: 20 weeks
Shift: Day (5x8 hours), 8:00 AM – 5:00 PM
Hours per Week: 40
Job Description
- Provide home health physical therapy services with a patient population including cardiopulmonary, geriatrics, neurological, orthopedic, and oncology specialties.
- Conduct skilled home visits, typically 6-8 visits per day (minimum 6 points per day, adjustable by manager).
- Responsibilities include assessment, cardiopulmonary/circulatory care, documentation, use of equipment and devices (e.g., crutches, gait belt, hospital bed, walkers, supplemental oxygen), orthopedic and neurological care, home exercise programs, medication management, patient and family education, abuse recognition (physical, sexual, psychological), and safety/injury prevention.
- Visits and hours are self-scheduled based on office needs.
- Every 3rd weekend work is required.
- Use of laptop, MiFi for internet access, and portable printers provided.
Required Qualifications
- Valid Physical Therapist license in Florida
- Physical Therapist (2 years of experience)
- Physical Therapist experience within the last year
- BLS issued by American Heart Association
- Experience in a Home Health setting
- Driver's License
- Cardiopulmonary/Circulatory assessment and treatment
- Orthopedic physical therapy
- Neurological physical therapy
- Home Exercise Program development and instruction
- Recognition of signs and symptoms of physical, sexual, and psychological abuse
- OASIS training
- Valid driver's license
- Personal vehicle for home visits
Compliance & Documentation Requirements
- Background checks including nationwide and county-level searches, sex offender registry, education verification, and EPLS/GSA/SAM checks within 1 year of start.
- Florida AHCA Level II fingerprint clearance required prior to start; must be current and on file.
- Primary source verification of state license required.
- Physical exam within 6 months of start.
- Negative TB test (PPD, Qgold, or Tspot) within 10 weeks of start; if prior positive, chest X-ray within past year plus completed TB questionnaire. TB questionnaire required annually in lieu of annual TB test.
- Immunizations or titers required for: Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B (titer, series, or declination accepted).
- Standard 9-panel drug test within 30 days of start (instant tests not accepted).
- Annual respirator medical clearance if respirator use is required.
- Completion of BayCare-specific online learning modules prior to start (approx. 4-8 hours).
- Photo for badge with solid background required prior to start.
- Personal or auto insurance must be current and active during assignment.
- Compliance with BayCare dress code and tobacco policy.
- Completion of BayCare Health MSP forms including privacy policy, affidavit, demographic information sheet, fit mask acknowledgement, and hurricane exemption survey if applicable.
Additional Requirements
- Two professional references from within the past 2 years.
- Employment application and skills checklist must be submitted and available for review.
- Medication/pharmacology competency exam required as applicable.
- Color blindness test required prior to start.

- Prior travel experience is not explicitly required for this position.
