Description
Physical Therapist - Home Health (Travel/Contract)
Location: Kingman, Arizona
Shift: Monday to Friday, 8:00 AM – 4:30 PM (40 hours/week)
Contract Length: 13 weeks
Job Description
- Provide physical therapy services in a home health/outpatient setting.
- Patient diagnoses include:
- Cerebrovascular Accident (CVA)
- Heart Failure (HF)
- Type 2 Diabetes Mellitus (DMII)
- Status post amputation
- Parkinson’s disease
- Paraplegia
- Dementia
Required Qualifications
- Valid Physical Therapist license in Arizona.
- Minimum 1 year of Physical Therapist experience, including experience within the last year.
- Experience in home health and outpatient settings.
- Experience treating patients with cerebrovascular accident (CVA), heart failure, amputation, Parkinson’s disease, paraplegia, and dementia.
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
Compliance and Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID).
- Background checks including:
- 7-year search within 30 days of start (updated annually) covering SS trace, county searches for last 7 years, nationwide, sex offender, and education verification.
- EPLS/GSA/SAM and HHS/OIG checks (required annually).
- Education verification and transcript/diploma required.
- Certifications as applicable: ACLS, PALS, NRP (per role requirements).
- Medication test/pharmacology competency exam and other competency exams as applicable.
- Physical exam within 1 year of start.
- Negative TB test or chest X-ray within 1 year of start; annual TB questionnaire required.
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
- 9-panel drug screen within 30 days of start (instant drug screen acceptable).
- Seasonal flu vaccination or declination during flu season (October 1 – March 31).
- Two professional references within the last 3 years.
- Annual skills checklist.
- Facility-specific documents including photo for badge, security/confidentiality agreements, behavioral expectations, HIPAA and information security training, and agency attestation forms.
Additional Information
- No on-call responsibilities.
- Work schedule is consistent day shift, Monday through Friday.
