Physical Therapist - Home Health (Travel/Contract)

Location: Gloucester, Virginia
Contract Length: 13 weeks

Job Details

• Employment Type: Travel/Contract
• Shift: Day shift, Monday to Friday (5x8 hours)
• Hours per Week: 40 hours (8:00 AM - 4:30 PM)
• No on-call responsibilities
• Mileage reimbursed at IRS rate
• Locals allowed at the same rate
• Must be proficient in OASIS documentation system

Required Qualifications

• Minimum 1 year of home health experience after graduation (time during school before licensure does not count)
• Valid and current driver’s license required
• Government-issued photo ID required
• Must meet all state licensure requirements for Physical Therapists in Virginia
• Must pass a 7-year criminal background check, including all locations lived/worked in the past 7 years
• Must have a clean driving record from the state of license issuance with at least 3 years driving experience; minimum age 21 (25 if transporting patients)
• Drug screen within the last 12 months required
• Physical exam within the last 12 months required
• Required immunizations/vaccinations or approved exemptions:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (series, titer, or declination)
  • Seasonal flu vaccination during flu season (10/1 - 3/31) or approved exemption
  • COVID vaccine series preferred; declination form accepted if no vaccine records available
• Tuberculosis screening: Negative TB test or chest X-ray within last 12 months; if positive, clearance letter signed by physician required
• Must complete facility-specific documents including confidentiality agreement, dress code policy, safety brochure, scrub color policy, drug-free policy, and hand washing hygiene policy
• Must provide a recent professional photo for badge (requirements specified)
• Two professional references required
• Application/resume required

Competencies & Certifications

• Current Basic Life Support (BLS) certification required (AHA, ARC, Military, or American Safety & Health Institute; online CPR not accepted)
• Other certifications (e.g., ACLS, PALS, NRP) not specified as required for this role; verify if applicable
• Must complete competency exams as applicable within one year of start date
• Pass/fail testing (e.g., EKG/medication) will be administered during orientation

Additional Notes

• E-Verify or attestation required depending on employment status
• Fingerprinting for criminal background check will transition to Virginia State Police National Criminal Background Check starting 7/15/2025
• Facility requires annual updates for some documents and screenings (e.g., TB questionnaire, background check)
