Job Description
- Outpatient Speech-Language Pathologist position
- Adult outpatient experience required
- Preferred experience with language, cognition, dysphagia, brain injuries, apraxia of speech, voice disorders, Parkinson’s disease, and pediatrics
Required Qualifications
- Valid Speech Language Pathologist license in Michigan
- Minimum 1 year of Speech Language Pathologist experience within the last year
- Basic Life Support (BLS) certification (American Heart Association or RQI accepted)
- ASHA Certification
- Experience working with adult patients
- Experience in an outpatient setting
Required Documentation and Compliance
- Valid Driver’s License or State ID (must be current throughout assignment)
- Social Security Card
- E-Verify compliance
- Background check within 30 days of assignment start, including:
- 7-year Social Security Number Trace
- National Criminal History Search (7 years)
- National Sex Offender Database Search
- FACIS Level 3 Search
- Education verification (highest level)
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- High school diploma/GED verification (not applicable if RN)
- Physical exam within 12 months of start (must note latex sensitivity)
- N95 Fit Mask test within 12 months of start (annual update required)
- Tdap vaccine within 10 years or declination
- Negative TB test or chest x-ray per policy
- Immunity documentation or vaccination for Measles, Mumps, Rubella (MMR), Hepatitis B, Varicella
- 10-panel drug screen with THC within 30 days of start (no medical marijuana cards or dilute screens accepted)
- Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
- Completion of facility-specific documents and orientation modules prior to start
- Two professional references from previous employers
- Current resume with at least 7 years of work history
- Skills checklist and competency exams as applicable
Additional Notes
- Fingerprinting via Michigan LARA required only for certain locations (not specified for this role)
- Orientation safety manual acknowledgment required
- No weekend or floating shifts
- Modules and facility-specific paperwork completed onsite at orientation
