Registered Nurse – LTC (Long-Term Care)

Location: Vestal, New York
Employment Type: Travel/Contract
Contract Length: 16 weeks


Job Overview

  • RN Supervisor role in a 300-bed long-term care facility
  • Patient population includes Alzheimer's & Dementia patients
  • Shift: Day shift, 8 hours per day, 5 days per week (6:30 AM – 3:45 PM)
  • Every other weekend required
  • Nurse-to-patient ratio approximately 1:18-20
  • Start dates every 2 weeks due to orientation schedule
  • No shift changes or request time off (RTO) allowed once offer is accepted
  • No medication passes required

Required Qualifications

  • Valid RN license in New York
  • Minimum of 1 year of experience in Long Term Care within the last year
  • Experience in Supervisor roles in a Long Term Care setting
  • BLS certification issued by American Heart Association, American Red Cross, ASHI, or National CPR Foundation
  • Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)

Schedule & Attendance

  • 8-hour shifts, 5 days per week
  • Every other weekend required
  • RTO only approved at time of submission; no additional RTO after offer
  • Call-outs require more than 24 hours’ notice

Facility & Work Environment

  • State/Government-run long-term care center offering rehabilitation therapies, skilled nursing, memory care, wound care, and more
  • Attire: Any solid color scrubs and closed-toe shoes
  • Charting system: PCC (Point Click Care) – no submission required
  • Free parking available
  • No mileage reimbursement

Onboarding & Compliance

  • Compliance process takes 1.5 to 2 weeks
  • Relias training modules must be started immediately upon receipt and completed at least 3 days before assignment start
  • Must pass all assigned modules with a minimum score of 80%; medication administration exam requires 90% passing score
  • Additional modules may be assigned after start date and must be completed by assigned due dates

Required Documentation & Health Requirements

  • Government-issued photo ID (front copy only)
  • Background checks including 7-year history, EPLS/GSA/SAM, and HHS/OIG (updated annually)
  • Physical exam within 1 year of start, updated annually
  • Negative TB test or chest X-ray within 1 year (exceptions apply for positive TB history or BCG vaccine)
  • Immunizations or titers required: Measles (Rubeola), Mumps, Rubella (2 vaccinations or positive titer; booster if low titer)
  • Flu vaccine or declination required annually; mask required if declined during flu season
  • COVID-19 vaccination or declination required
  • Pneumonia vaccination proof or declination required
  • Drug screening (9-panel) required for direct hires within 1 year, not required annually
  • Harassment prevention training certificate required for subcontractors
  • Two professional references required (supervisors preferred)
  • Resume or employment application required
  • New York State Immunization Information System (NYSIIS) consent form required

Additional Notes

  • No floating between units required
  • No accommodation for block scheduling
  • Must provide proof of New York RN license at time of submission
  • Fingerprinting required for CNAs only (not applicable for RN role)
  • Candidates must complete all required documentation and training timely to avoid delays in clearance
