Description
Occupational Therapist - Home Health (Travel/Contract)
Location: Dunedin, Florida
Contract Length: 13 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 40 hours/week)
Job Description
- Provide home health occupational therapy services with a focus on:
- Activities of Daily Living (ADLs)
- Orthopedics
- Prosthetics/Orthotics
- Home Exercise Programs
- Patient and Family Education
- Safety and Injury Prevention
- Use of Evaluation Tools
- Billing and Documentation
- Conduct 6-8 skilled home visits per day (minimum 6 visits, may increase at manager’s discretion)
- Patient population includes amputees, arthritis, stroke rehabilitation, dementia/Alzheimer’s, hand injuries, traumatic brain injury, multiple sclerosis, nerve injury, Parkinson’s disease, spinal cord injury, tendon repair, and other progressive neurological disorders
- Visits and hours are self-scheduled based on office needs
- Use of laptop, MiFi for internet access, and portable printers provided
- Documentation through specific electronic systems
Required Qualifications
- Valid Occupational Therapist license in Florida
- Minimum 2 years of Occupational Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
- Valid driver’s license and reliable car for home visits
- Experience with Orthopedics
- Experience with Prosthetics/Orthotics
Compliance and Documentation Requirements
- Background checks including:
- Standard 7-year history check
- EPLS/GSA/SAM, HHS/OIG checks
- Foreign equivalency evaluation if education was completed outside the US or Puerto Rico
- Education verification and transcript/diploma required
- Florida AHCA Level II fingerprint clearance required prior to start (Livescan electronic fingerprinting)
- Physical exam within 6 months of start date
- Negative TB test (PPD, Qgold, or Tspot) within 10 weeks of start; chest X-ray and TB questionnaire if prior positive PPD
- Annual TB questionnaire required in lieu of annual TB test
- Immunizations or titers required for:
- Measles (Rubeola)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, series, or declination accepted)
- Standard 9-panel drug test within 30 days of start (instant tests not accepted)
- Respirator medical clearance annually if respirator use is required
- Two professional references from the past 2 years
- Employment application and skills checklist must be submitted
Additional Facility Requirements
- Professional photo with solid background required for badge issuance
- Current personal or auto insurance required for home health positions
- Compliance with dress code and tobacco policy
- Completion of required online account registration and training modules prior to start (approx. 4-8 hours)
- Completion of facility-specific forms including privacy policy, affidavit, demographic information, mask fit acknowledgment, and hurricane exemption survey if applicable
- Electronic signature form required for home health candidates
Competency and Certification Exams
- Medication/pharmacology competency exam with minimum 80% score
- Color blindness test prior to start
- Competency exams specific to home health occupational therapy as applicable
Notes
- Scrubs: Any solid color scrub allowed
- Candidates must maintain all licenses, certifications, and clearances current throughout the assignment
- Failure to maintain required certifications or clearances may result in removal from assignment
- Candidates with international education must provide foreign equivalency evaluation from a nationally recognized organization
This position offers an opportunity to provide specialized occupational therapy care in a home health setting with a diverse patient population requiring skilled rehabilitation services.
