OT Inpatient Rehab
Job Overview
- Location: Stillwater, Oklahoma
- Employment type: Travel/Contract

- Shift: Day, Monday–Friday, 8:00 AM–4:00 PM
- Weekly hours: 40
- Schedule note: 1 Saturday coverage per month
Required Qualifications
- Valid Occupational Therapist license in Oklahoma
- BLS issued by American Heart Association or American Red Cross
- 2 professional references within the past 3 years
Additional Requirements
- Government-issued photo ID
- E-Verify or attestation
- Background check with all locations lived/worked in the past 7 years; must be cleared before start and updated annually
- EPLS/GSA/SAM check
- HHS/OIG check
- Skills checklist
- Employment application
- Competency exam
- Medication/pharmacology competency exam, as applicable
- HIPAA acknowledgment
- Physical exam within 1 year of start
- Tdap vaccination within 10 years of start, or declination
- TB screening within 12 months of start
- Measles, mumps, rubella, varicella, and hepatitis B immunity/vaccination documentation
- Drug screen within 1 year of start
- Current COVID vaccination or COVID declination form
- Flu documentation or declination, if applicable
- Review and sign required orientation, confidentiality, and facility policy documents before start
- Passport-style photo for badge
Notes
- HealthStream modules must be completed before the Monday start date, by Friday afternoon
- First 16 hours of orientation are non-billable
