Role Overview

• RN case manager for home health in the Princeton Health South team, covering Plainsboro and West Windsor.
• High-volume home health caseload with 5–6 patients per day and substantial documentation.

Required Qualifications

• 2+ years of home health experience
• Case management experience
• OASIS experience
• Active RN license

Preferred Qualifications

• Wound care experience

Schedule

• 8:00 AM to 4:30 PM
• 40 hours per week
