Home Care Registered Nurse
Job Overview

Required Qualifications
- Valid RN license in Vermont
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS
- Home Health experience within the last year
- 2 years of Home Health experience
- Hospice experience
- Experience with OASIS
Key Responsibilities
- Assess patient and caregiver needs and develop a plan of care
- Serve as case manager and manage caseloads and visit times
- Complete OASIS documentation and other required forms accurately and on time
- Implement nursing interventions and procedures
- Provide patient and caregiver education
- Coordinate care with other disciplines and providers
- Maintain accurate nursing documentation
- Coordinate safe discharges and transfers
- Participate in peer review audits and case conferences
