Job Overview

Required Qualifications
- Valid CT Technologist license in Arizona
- CT Technologist experience within the last year
- One completed travel assignment
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
- ARRT
- Experience with Meditech
Documentation and Compliance
- Government-issued photo ID
- Background check with 7-year search within 30 days of start; must be updated annually
- EPLS/GSA/SAM check, annually
- HHS/OIG check, annually
- Transcript or diploma
- Education verification through NSCH
- Facility-specific job description
- Facility-specific forms and training, including confidentiality, behavioral expectations, right to know, HIPAA/information security, and agency candidate form
- Photo for badge
- Physical exam within 1 year of start
- Fit test within 1 year of start; updated annually
- Negative TB test or chest X-ray within 1 year of start; annual TB questionnaire required
- Measles, mumps, and rubella immunity documentation
- Hepatitis B documentation
- Varicella documentation
- 9-panel drug screen within 30 days of start
- Flu vaccination or declination during flu season
- 2 references within 3 years of start
- Skills checklist annually
Additional Notes
- Meditech charting required
- ACLS, PALS, and NRP are listed as applicable certifications
- Competency exam and medication/pharmacology competency exam are listed as applicable requirements
