Opportunity:

RN - Home Health

Job Details:

• Work 10-hour day shifts from 8:00 AM to 6:00 PM
• Two different shift patterns: Thursday-Friday-Saturday-Sunday or Saturday-Sunday-Monday-Tuesday
• 40 guaranteed hours per week for 13-week contract
• 1:1 patient ratio caring for adults 18 years and older
• Provide chronic disease management including CHF, COPD, Diabetes, Cancer, CVA
• Perform wound care and IV administration
• Travel requirements: 7-7.5 patient visits within 10-hour day, 50-100 mile radius
• No on-call or call-back requirements
• Required certifications: BLS (American Heart), Wisconsin RN License, Valid Driver's License
• Minimum 1-2 years current Home Health and Hospice experience required
• Recent OASIS experience required
• Epic charting experience required within 12 months
• Must be able to adapt to ever-changing schedule and work independently
• Need physical tolerance for driving throughout the day and navigating stairs/uneven landscapes
• Must inquire about allergies as home environments may have pets and/or smoke
• Work in various weather conditions
• Wear plain colored, well-maintained scrubs
• Associate degree or certificate required, Bachelor's degree preferred
• Holiday rotation requirements apply
• May float to other units and facilities based on census
