Opportunity:

RN - Home Health

Job Details:

• 13-week contract assignment with 40 guaranteed hours per week
• Work 10-hour day shifts (8:00 AM - 8:00 PM)
• Example schedule rotations: MTWF, MTW Sat, Sun, TWF
• Conduct 6 patient visits per day, spending approximately 1 hour per patient including drive time, patient visit, and charting
• Travel within 6 county service area covering approximately 250 miles per week (average 50 miles per day)
• 1:1 patient ratio working with adults 18 years and older
• Manage chronic diseases including CHF, COPD, Diabetes, Cancer, CVA, wound care, and IV administration
• No on-call or callback requirements
• Current home health and hospice experience required including OASIS experience
• Recent Epic charting experience required within 12 months
• Required certifications: BLS (American Heart), Wisconsin RN License, Valid Driver's License
• OASIS certification is a plus
• Must be comfortable with admission visits, death visits, and symptom management in hospice
• Must be able to physically tolerate driving throughout the day and navigate up/down stairs and uneven landscapes
• Need to inquire about allergies as home environments may have pets and/or smoke
• Visits must occur despite weather conditions
• Scrub colors: Any or corporate casual dress
• Float requirements based on census as needed
• Holiday rotation as needed
• Associate degree or certificate required, bachelor's degree preferred
