Opportunity:

RN - Home Health

Job Details:

• Work 5-8 hour variable shifts Monday through Friday
• Guaranteed 38 hours per week with flexibility up to 40 hours
• 7-week contract assignment
• Provide home health nursing services to 20-25 patients in Stevens Point and Plover areas
• Average 4.5 visits per day with travel ranging 25-60 miles daily depending on territory and patient census
• On-call requirements 1-2 times during the week and every other weekend
• Must respond to answering service calls within 15 minutes
• Weekend on-call and holiday rotation every other weekend
• Float to other territories when patient census is low
• Perform OASIS admissions, episode management, and coordinate patient care
• Handle wound care, IV therapy, port care, lab draws, and chronic disease management
• Complete documentation in EMR in timely manner
• Make doctor calls and coordinate care as needed
• Required certifications: AHA BLS, active Wisconsin or Compact license, valid driver's license
• OASIS certification required, Epic experience preferred
• Bachelor's degree required
• Minimum 2+ years home health experience preferred with travel experience required
• Must live outside 50-mile radius from facility
• Any scrub colors acceptable
• Mask required during flu season if not vaccinated
