Opportunity:

RN - Home Health

Job Details:

• Work 5-8 hour variable shifts Monday through Friday
• 38 guaranteed hours per week with flexibility to 40 hours
• 7-week contract assignment
• Provide care for 20-25 patients with average of 4.5 visits per day
• Travel 25-60 miles per day depending on territory and patient census
• On-call requirements: 1-2 times during the week and every other weekend
• Must respond to answering service calls within 15 minutes
• Weekend and holiday rotation every other weekend
• Float to other territories when needed due to low patient census
• Perform OASIS admissions, episode management, and coordination of care
• Required skills include wound care, IV management, ports, lab draws, and chronic disease management
• Chart in EMR system with timely documentation requirements
• Must live outside 50-mile radius from facility
• Any scrub colors acceptable
• Bachelor's degree required
• Minimum 2+ years home health experience preferred with travel experience required
• Required certifications: BLS (AHA or ARC), active Wisconsin or compact nursing license, valid driver's license
• OASIS certification required, Epic experience preferred
• During flu season, unvaccinated travelers must wear masks at all times
• Contract eligible for 36 hours of unbillable low census time
