Job Details:

• Work as a home health occupational therapy case manager
• Manage a caseload of 14 home health occupational therapy patients
• Provide occupational therapy services in patients' homes
• Coordinate and manage patient care plans
• Document patient progress and treatment outcomes
• Collaborate with healthcare team members and families
• Travel to various patient locations for service delivery
