PT - Physical Therapist

Job Details:

• Work as PT Case Manager
• Provide physical therapy services across multiple coverage areas
• Coverage area includes: Cicero, Berwyn, Stickney, Riverside, Lyons, Brookfield, La Grange, La Grange Park, Broadview, Westchester, Maywood, Bellwood, Melrose Park, Stone Park, Northlake, Hillside, Elmhurst, and Western Springs
• Travel required between various locations within coverage area
• Case management responsibilities included in position
