Opportunity:

RN - Home Health

Job Details:

• Work as a field hospice nurse providing home-to-home patient visits
• Must be able to work independently in the field
• Assigned daily visits for end-of-life care patients
• Additional as-needed visits will be added to schedule throughout the day
• Must be able to travel between patient homes
• Serve Burlington/Walworth area
• Position requires reliable transportation for field work
• Provide supportive care as part of hospice team
