Opportunity:

RN - Home Health

Job Details:

• Work 5-8 hour variable shifts Monday through Friday
• Guaranteed 38 hours per week with flexibility to 40 hours per week
• 13-week contract with ASAP start date
• Provide home health services to 20-25 patients with approximately 4.5 visits per day
• Travel 25-60 miles per day depending on territory and patient census
• On-call requirements: 1-2 times during the week and every other weekend
• Weekend and holiday rotation every other weekend
• 15-minute response time required for answering service calls
• Float to other territories when needed due to low patient census
• Perform OASIS admissions, all time points, and episode management
• Handle doctor calls and coordination of care with timely EMR documentation
• Clinical competencies required in wound care, IV therapy, ports, lab draws, and chronic disease management
• Case management responsibilities for Stevens Point/Plover area patients
• Any scrub colors acceptable
• Required certifications: AHA BLS, active Wisconsin or Compact nursing license, valid driver's license
• OASIS certification a plus
• Minimum 2+ years home health experience preferred, travel experience required
• OASIS charting experience required, Epic preferred
• Bachelor's degree required
• Must live outside 50 miles from facility location
