Opportunity:

RN - Home Health

Job Details:

• Work after hours shift from 4:00 PM to 12:00 AM
• Field-based position providing hospice patient care
• Coverage areas include Sheboygan/Manitowoc or Green Bay depending on facility needs
• Patient visits are call-based and primarily focus on symptom management
• Respond to death calls as needed
• Travel required between patient locations within assigned coverage area
• Must be comfortable providing end-of-life care and symptom management
• Ability to work independently in field settings required
