Opportunity:

RN - Home Health

Job Details:

• Work 8-hour varied shifts with 37 guaranteed hours per week and flexibility up to 40 hours
• Every 4th weekend required with holiday rotation
• On-call responsibilities 1-2 times per week and every other weekend
• 15-minute response time required for on-call situations
• Care for 20-25 patients with approximately 4.5 visits per day
• Travel approximately 50 miles per day depending on territory and patient census
• May float to other territories to assist when needed during low census
• Minimum 2 years of home health experience with travel experience required
• Required certifications: Wisconsin nursing license, BLS (AHA or ARC), valid driver's license
• OASIS certification preferred, EPIC experience preferred, OASIS experience required
• Perform OASIS admissions, episode management, doctor calls, and care coordination
• Maintain clinical competencies in wound care, IV therapy, port access, lab draws, and chronic disease management
• Complete documentation in EMR in a timely manner
• Associate degree or certificate education required
• Contract eligible for 36 hours of unbillable low census hours
• May be placed on call during low census with minimum 2-hour call back pay
