Opportunity:

Physical Therapist - Physical Therapist

Job Details:

• Work Monday through Friday, 8:00 AM to 5:00 PM schedule
• Position is 100% remote within designated coverage area after initial orientation
• Approximately 2 days onsite orientation in Oak Brook, IL
• Expected to see 6 patient visits per day within coverage area
• No weekend rotation, call, or holiday expectations required
• Work with adult/geriatric general medical/surgical type patients
• Current census of approximately 200 patients
• Must provide own transportation to/from all patient visits
• Valid driver's license and auto insurance required
• Coverage area: Central Metro E (zip codes 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas)
• Required certifications: Active Illinois PT license, BLS (AHA), master's degree in PT
• Minimum 2 years PT home health experience with adult/geriatric patients required
• Must be able to assist with ADLs and adaptive equipment
• Must effectively collaborate with other specialties (RN, OT, Speech, etc.)
• All meetings held virtually with supplies and equipment delivered to home
• Dress code: Any solid color scrubs or business casual attire acceptable, no jeans or open toe shoes
• EMR system: EPIC
