Opportunity:

RN - Home Health Nurse

Job Details:

• 8-hour varied shifts with every 4th weekend required
• Holiday rotation required
• Guaranteed 37 hours per week with flex up to 40 hours per week
• On-call responsibilities 1-2 times during the week and every other weekend, primarily involving triage calls with a 15-minute response time requirement
• On-call duties include determining if patient concerns can be handled via phone or require an in-person visit (e.g., foley catheter troubleshooting, wound vac issues)
• Average of 4.5 patient visits per day with approximately 20-25 patients and 50 miles of travel per day depending on territory and patient census
• Potential to float to other territories to assist during low census periods
• Responsibilities include OASIS admissions, all time points, episode management, physician calls, coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Minimum 2 years of home health experience with travel experience required
• Associate Degree or Certificate required
• Required certifications: Wisconsin RN License, BLS (AHA or ARC), Valid Driver's License
• OASIS certification is a plus; experience with Epic Remote Client/Remote Care preferred
• Strong time management, dependability, adaptability, and compassionate patient care skills expected
