Opportunity:

RN - Home Health

Job Details:

• Work 13-week contract with guaranteed 40 hours per week
• 8-hour day shifts from approximately 8:00 AM to 4:30 PM with potential flexibility based on census
• Every 3rd weekend and holiday rotation required
• On-call duties every 3rd weekend with possibility of increased coverage based on nursing pool
• Average 5 home visits per day, potentially up to 6 visits depending on territory and patient census
• Travel requirements may include up to 150 miles in a given day
• Perform Oasis admissions, all time points, episode management, and coordination of care
• Conduct doctor calls and maintain timely EMR documentation
• Clinical competencies required in wound care, IV therapy, ports, lab draws, and chronic disease management
• 2+ years home health experience preferred
• Required certifications: AHA BLS, active Michigan and Wisconsin licenses, valid driver's license
• Charting experience: Oasis required, Epic preferred
• Associate degree or certificate required, bachelor's degree preferred
• Contract eligible for 36 hours of low census unbillable hours
• Must live outside 50-mile radius from facility
