$500 CONTRACT COMPLETION BONUS
Opportunity:
RN - Home Health
Job Details:
• Work weekend program covering Friday, Saturday, and Sunday shifts from 8:00 AM to 8:00 PM
• Night shift available from 8:00 PM to 8:00 AM, 3 shifts per week
• Home hospice position requiring travel to patients' homes throughout assigned territory
• Patient ratio of 0-6 patients depending on call-ins for the night
• Minimum 2 years of home health or home hospice experience required
• Case management experience in home setting preferred for first-time travelers
• Required certifications: BLS, active driver's license, Wisconsin RN license, own vehicle, and auto insurance
• Navy blue scrubs required
• Provided equipment includes badge, laptop, cell phone, and trunk kit
• Perform focused assessments, troubleshoot feeding tubes, wound care, post-fall assessments, and death visits
• Comfortable administering narcotic pain medications and identifying potential drug diversions
• Handle after-hour visits called into triage line for symptom management, falls, and death visits
• Work with terminally ill patients including advanced cancers, end-stage COPD, and failure to thrive
• Use Epic EMR system (Rover) and Remote Client technology
• May occasionally float outside assigned territory if closer to emergency calls
• Holiday expectations include 1 summer and 1 winter holiday
• 3-shift orientation (1 week) with SCT nurse and NEO at territory office
• 24/7 Lead RN support available for consultation and reporting
