Opportunity:

RN - Home Health Nurse

Job Details:

• Work Monday through Friday, 8:00 AM to 4:30 PM (end time may vary)
• Weekend shifts only required during on-call rotation
• On-call requirement: 2 times per week, Monday through Thursday
• Post-acute care setting serving primarily geriatric population with occasional pediatric patients
• See 5-6 patients per day depending on type of visit, with 25-30 points per week including routine and OASIS visits
• Provide both home health and hospice visits - no floating to other departments
• Common diagnoses include heart disease, CHF, COPD, pneumonia, dementia/Alzheimer's, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, and surgery
• Perform medication management, catheter management, NPWT, ostomy care, central line care, IV administration, and complex wound care
• Provide hospice care including pain and symptom management, end-of-life care with empathy and compassion
• No ventilator management required; CPAP and BiPap used occasionally and self-managed by patients
• Participate in weekly IDT meetings and case conferences at office, as well as monthly all-staff meetings
• Administrator available after hours, weekends, and holidays for support
• Charting system: Cerner
• Orientation includes 1 day of computer-based training and 2-5 days of field training and shadowing
• Dress code: Solid color scrubs with no patterns or logos unless company-issued; closed-toe, non-slip shoes with socks or hosiery required; no flip-flops, open-toe shoes, or sandals permitted
• Required certifications: BLS (AHA)
• Travel experience, critical thinking abilities, and chronic disease management experience preferred

This post-acute healthcare facility operates a Home Health and Hospice department providing compassionate care to patients in their homes. The department is supported by a team that includes certified nursing assistants and is managed by an administrator who remains accessible after hours, on weekends, and during holidays. The facility utilizes Cerner as its charting system and is committed to delivering high-quality care across both home health and hospice services.
