Opportunity:

Social Worker - Hospice

Job Details:

• Monday through Friday schedule, 8:00 AM to 4:30 PM
• No weekend rotation, on-call requirements, or holiday expectations
• Serve a census of approximately 30 patients in home and facility settings
• Conduct 4-5 patient visits per day with autonomous scheduling
• Patient ratio of 1:35 (MSW)
• Focus on end-of-life care with no pediatric patients
• Float throughout the Manitowoc and Green Bay region as needed
• Support provided by 2 other social workers in different markets, a Lead RN, and a Hospice Medical Director
• Minimum 1 year of social work experience required; hospice experience highly preferred
• First-time travelers accepted if they have hospice social work experience
• Required credentials: MSW degree, LCSW state license, and valid Driver's License
• Hospice social work experience required
• EMR: EPIC Remote Client experience highly preferred
• Dress code: Scrubs of any color or business casual attire

This hospice facility serves the Manitowoc and Green Bay region, providing compassionate end-of-life care to patients in both home and facility settings. The organization maintains a supportive team environment with dedicated clinical leadership, including a Hospice Medical Director and Lead RN, ensuring social workers have the resources and guidance needed to deliver high-quality patient care. The facility utilizes EPIC as its electronic medical records system and provides a structured orientation program including departmental training and an in-person preceptor experience.
