Opportunity:

RN - Home Health Nurse

Job Details:

• Work 8-hour day shifts with a daily huddle around 8:15 AM and typical start time of 8:00 AM
• Monday through Friday schedule with variable shifts
• Guaranteed 38 hours per week with flex up to 40 hours per week
• Every 3rd weekend and every 3rd holiday rotation required
• On-call required 5 times per month, primarily for triage; the clinician working the weekend for admissions/routines will also be on-call for their assigned weekend
• Must respond to answering service calls within 15 minutes for triage; additional visits may be required for issues such as blocked Foley catheters or wound vac malfunctions
• See 4-6 patients per day depending on visit type, with travel ranging from 25-60 miles per day depending on territory, patient census, and staff availability
• Productivity benchmark of 1.95 hours per visit
• Patients seen in the Stevens Point and Plover area
• May float to adjacent territories based on census; no long-distance or non-adjacent floating
• Expected to assist in case managing Stevens Point/Plover area patients as needed
• Perform OASIS admissions and all time points, episode management, doctor calls, and coordination of care with PT/OT and other disciplines
• Clinical competencies required include wound care and wound vacs (KCI/3M primarily; some Medela), IV therapy including PICC lines and ports for IV antibiotics, lab draws, enteral feeding (G-tubes and J-tubes), urinary catheters (Foley and suprapubic), ostomy care including new ostomies, and chronic disease management (e.g., diabetes, HF, CAD)
• Minimum 2+ years of recent home health RN experience required; previous travel assignment required
• Case management experience highly preferred
• OASIS experience required; experience with Epic Remote Client/Remote Care required
• OASIS certification a plus
• Associate Degree or Certificate required
• Required certifications: Active WI or Compact RN license, valid driver's license, and AHA or ARC BLS certification
• Scrub colors: Any
• Four-day orientation provided
