Job Summary: Home Health Psych Registered Nurse (HH PSYCH) South Dennis, Massachusetts
The organization is seeking a Registered Nurse to perform comprehensive nursing assessments upon patient admission, incorporating physiological, physiopsychological, and environmental factors along with health history to support safe, quality care. The role integrates assessment data to identify problems that form the basis of a plan of care, evaluates patient needs, and documents clinical outcomes while coordinating with interdisciplinary teams. Candidates are required to bring their license on the first day.

Job Details:

• Schedule: Days shift with 8 shift hours; minimum 40 hours per week.
• Required Qualifications: Current Registered Nurse (RN) license in Massachusetts and other related credentials to meet specialty; CPR (Cardiopulmonary Resuscitation) certification; successful passage of VNA pre-hire medication test; minimum one (1) year of nursing experience required or 6 months of nursing experience with a BSN (Bachelor of Science in Nursing).
• Preferred Qualifications: Home care and IV experience; computer skills; BSN (Bachelor of Science in Nursing).
• Responsibilities:
  • Conduct comprehensive nursing assessments upon admission, including physiological, physiopsychological, and environmental factors and health history.
  • Integrate assessment findings to identify problems that form the basis of the plan of care and adhere to case management objectives.
  • Evaluate clinical needs, initiate, develop, implement, and revise the patients plan of care as needed; address and document all problems in the plan of care.
  • Participate in interdisciplinary patient case conferences and evaluate the effectiveness of the established plan of care.
  • Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
  • Notify MD (Doctor of Medicine) and other involved care providers of significant changes in patient condition.
  • Complete admission and other paperwork on a timely basis; record narrative documentation reflecting skilled care with clear, concise entries addressing teaching, direct care, and patient/caregiver response.
  • Obtain and clarify valid MD orders for all care delivered.
  • Plan and coordinate discharges appropriately; inform patient/MD and document disposition of the patient in the discharge summary.
  • Complete all appropriate discharge paperwork.
  • Maintain annual HHRG values at the National Standard as appropriate and use those values to establish visit frequency and interdisciplinary utilization; ensure annual outcomes meet National Benchmarks; refer to community resources as appropriate.
  • Perform other duties as assigned.
