The Home Health Registered Nurse is responsible for providing and documenting skilled one-on-one nursing care in accordance with the physician-ordered individualized patient care plan.  This care is provided mostly in the patient’s home or a residential setting but can transfer outside the home during appointments and other activities the nurse may accompany the patient to.  Minimum Requirements: <ul><li>Graduate of an accredited school of nursing</li><li>Currently licensed as a Registered Nurse in the state in which the RN will practice</li><li>Current CPR if applicable</li><li>TB questionnaire, PPD or chest x-ray if applicable </li><li>Current Health certificate (per contract or state regulation) </li><li>Must meet all federal, state and local requirements </li><li>Must be at least 18 years of age </li></ul> Successful passage of VNA pre-hire medication testCurrent RN license in Mass. and other related credentials to meet specialty.Minimum one (1) year of nursing experience required or 6 months of nursing experience with a BSN.CPR certification required.Home care and IV experience, computer skills and BSN preferred.
