Home Health Nurse Manager (Home Health) Clearlake, California. This role involves planning, organizing, and directing home care services to meet patients' needs in their homes. Job Details:

• Schedule: Dayshift, 12-hour shifts, minimum of 36 hours per week
• Required Qualifications: Bachelor's Degree in Nursing (BSN); preferred experience in acute care facility or home care setting; Registered Nurse (RN) licensure in the state of practice; Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor; valid Driver's License (DL) with a reliable vehicle and current automobile insurance; must be at least 21 years of age; 1-2 years of experience in home health care; must have HCHB software experience and OASIS experience.
• Additional Information: The role involves on-call duties as assigned, depending on location and operational needs. Candidates will perform comprehensive assessments, coordinate multidisciplinary care, develop and update care plans, document all patient interactions accurately, and educate patients and families about healthcare needs. The position is based in Clearlake, California, with a coverage area in Lake County.
