Job Summary:
Hospice Registered Nurse (RN) Basking Ridge, New Jersey. This position involves providing comprehensive hospice care through patient assessments, developing care plans, and ensuring the appropriate utilization of services. The RN will collaborate with interdisciplinary teams, educate patients and families, and maintain compliance with hospice regulations and insurance guidelines to deliver high-quality care.

Job Details:

• Schedule: Days shift, 7.5 hours per day, minimum of 37.5 hours per week
• Required Qualifications: Current New Jersey RN license, associates degree in nursing or diploma with RN license, minimum of 2 years of RN experience in a home hospice environment, experience with electronic medical records (EMR), excellent critical thinking and communication skills, valid New Jersey drivers license, BLS certification
• Preferred Qualifications: Over 3 years of RN experience in hospice care
• Additional Information: Performs patient assessments, develops and revises care plans, manages medication reconciliation and safety, participates in interdisciplinary team meetings every 2 weeks, contributes to quality improvement activities, provides mentorship to new staff, and ensures safe, effective patient care in accordance with hospice standards and regulations.
