Job Summary:
Home Healthcare Palliative Care Case Manager position located in Martinez, California. The role involves providing patient visits in their homes, skilled assessments, and reporting to practitioners or medical doctors. The position requires a candidate capable of managing clinical responsibilities and administrative tasks related to home health service delivery.

Job Details:

• Shift: Days, 8 hours per day, Monday through Friday, with a minimum of 40 hours per week.
• Required Qualifications: Palliative Care Case Manager with at least 1 year of recent full-time home health experience within the last 3 years; valid license and certifications, including Basic Life Support (BLS).
• Additional Information: The assignment is expected to last for 13 weeks and will be based onsite in Martinez, with visits to patients in their homes, skilled nursing facilities, or assisted living communities. The work involves clinical charting distinct from Oasis, with assessments and SBAR reporting to practitioners or physicians. There is no current plan for winter coverage.
• Float: Float rotation may be required.
